Reflections from a Primary Care Physician

I think In primary care we continue to plummet and although the media says it is strengthening, objective data says the opposite. This is what I explained to my integration bureau the day before yesterday.

Rather than empowering us, the measures taken this year in primary care have further trapped us:

• The new dependency report continues to be bureaucratic, cumbersome, and does not improve upon the previous report.
• New casualty module, same or worse. 10 months later, they have not eliminated any sick leave, and hospital-generated sick leave continues to be forced onto the gum agenda.
• Having an OPE in mid-July may mean they are unable to cover absences.
• The current new agenda is that 70% of face-to-face appointments are canceled due to absences as an election promise so that “we stop diagnosing bronchitis over the phone”. Our consultation is fraught with bureaucracy, much of it generated by second tier, and generated by councils whose protocols overload primary care.
• They host EIRs outside health centers and send nurses with no primary care experience to the centers. The same goes for AAC staff.
• They know they can’t replace them, and they require medical staff to work on piece rates, so they limit the issuance of licenses to January 7.
• They still don’t have a JUAP version, but they have loaded tasks (Bikain, complex computer tools with unlimited indicators, Osakidetzakoa…)
• They hope to encourage the use of the Osabide combination, and not a day goes by without a crash when the drug is updated.

Every new measure they implemented was shuddering.
